Cominciò a rifiutare il cibo il 1° marzo dell'81 e finì col morire nelle prime ore del 5 maggio mentre la sua scomparsa veniva accompagnata dal suono delle bottigle molotov lanciate dai dimostranti e dallo schiocco delle pallottole di plastica sparate dalla polizia e dall'esercito a Belfast. Il funerale di Sands costituì una tra le più grandi dimostrazioni politiche a memoria d'uomo con migliaia di nazionalisti provenienti da tutto il paese.
